In the F Scott Fitzgerald story 'May Day' (1920), two drunk revellers in an elevator - 'Mr In' and 'Mr Out' - tell the operator that they want to go to 'the top floor'. When told that they are already there, they demand to go even further upwards: 'Have another floor put on!'; 'Higher!'; 'Heaven!'
